External Rotation

External Rotation

After the delivery of the fetal buttocks, the fetal sacrum may rotate posteriorly towards the maternal sacrum instead of anteriorly. If this is the case an external rotation maneuver needs to be performed to rotate the fetal sacrum anteriorly. The thumbs of the healthcare professional are placed parallel to the fetal spine on the sacrum. The other fingers grasp the fetal pelvis and the upper legs.

Take care not to hold the fetus by the flanks or abdomen as this may cause injury to the kidneys or liver.
